Program Terms and Condition

Funda Institute Occupational Health Program - Terms and Conditions

By applying for the Funda Institute Occupational Health Program, applicants agree to abide by the following terms and conditions:

1. Eligibility Criteria

  • Residency Requirement: Applicants must reside in one of the following areas:

    • Tweefontein

    • Moloto

    • Kwamhlanga

    • Kwaggafontein

    • Mathys Zyn Loop

    •  

  • Proof of Residency: Applicants are required to provide valid Proof of Residency with their application. Acceptable documents include:

    • Certified utility bill not older than three months.

    • Municipal letter confirming residence.

    • An affidavit signed by a commissioner of oaths confirming residence.

2. Application Process

  • Complete Applications: All required fields in the application form must be completed accurately.

  • Documentation: Failure to submit valid Proof of Residency and other required documents will result in disqualification.

  • Deadlines: Applications must be submitted by the specified deadline. Late submissions will not be accepted.

3. Program Participation

  • Attendance: Full attendance is mandatory for all scheduled classes, practical sessions, and program activities.

  • Assessments: Participants must complete all assignments, exams, and practical assessments to the satisfaction of the program requirements.

  • Conduct: Participants are expected to adhere to the Funda Institute's code of conduct and maintain professional behavior at all times.

4. Graduation

  • Compulsory Graduation: Graduation is mandatory for all participants who successfully complete the program.

  • Graduation Fees: Participants are responsible for paying their own graduation fees, which cover:

    • Graduation attire (gown, cap, and hood).

    • Venue and ceremony costs.

    • Administrative fees related to certification.

  • Payment of Fees: Graduation fees must be paid in full by the specified deadline prior to the ceremony.

  • Certificate Issuance: Certificates will only be issued to participants who attend the graduation ceremony and have paid all associated fees.

5. Certificates

  • Eligibility: Only program graduates who have fulfilled all program requirements, including attending the graduation ceremony, will receive certificates.

  • Collection of Certificates: Certificates will be distributed during the graduation ceremony. Those unable to attend must make arrangements with the program office, but certificates may be withheld until graduation requirements are fulfilled.

6. Reporting to Sponsors

  • Graduation as Sponsor Report: The graduation ceremony will serve as a formal report to program sponsors, showcasing the successful completion of the program by participants.

  • Participation: By attending the graduation, participants consent to being included in reports, photographs, and promotional materials shared with sponsors.

7. Financial Obligations

  • Program Costs: While the program tuition is provided at no cost to participants, they are responsible for graduation fees as outlined above.

  • Refunds: Graduation fees are non-refundable. Failure to attend the ceremony without an approved exemption will result in forfeiture of fees paid.

8. Data Protection and Privacy

  • Consent: Applicants consent to the collection and use of their personal information for purposes related to program administration, certification, and reporting to sponsors, in accordance with Funda Institute’s POPIA Policy.

  • Media Release: Participants agree that photographs and videos taken during the program and graduation may be used for promotional purposes on social media, marketing materials, and reports to sponsors.

9. Non-Compliance

  • Disqualification: Non-compliance with any of the terms and conditions may result in removal from the program and ineligibility to receive a certificate.

  • Disciplinary Action: The institute reserves the right to take appropriate disciplinary action for misconduct or breach of these terms.

10. Communication

  • Updates and Notices: All official communication will be sent via the contact details provided in the application. Participants are responsible for checking their emails and messages regularly.
